The first two weeks home from the hospital are the most critical. We take immediate control of the discharge plan, audit medications, and closely monitor healing to prevent complications and keep your loved one out of the emergency room.

What’s Included in the 14-Day Protocol:

  • The 48-Hour Critical Intake (In-Home)

    An immediate 60-minute evaluation upon returning home, including complete medication reconciliation, vital baseline checks, and a home safety audit.

  • 2 Follow-up Clinical RN Visits (In-Home)

    Strategically timed to monitor wound healing, check vital signs, and ensure the recovery trajectory is on schedule.

  • 2 Virtual RN Check-ins

    Scheduled touchpoints to ensure medication compliance, address new symptoms, and answer family questions.

  • 14-Day Non-emergent virtual Triage & Coordination (8am-Midnight)

    Your dedicated emergency-trained RN knows your health story & provides scheduled virtual consultations for non-emergency triage & care coordination, 8 a.m.–midnight

  • Day 14 Care Plan Transition

    A final assessment wrapping up the critical period with clear ongoing recommendations for long-term stability.
